FT Care Coordinator/Case Mgmt position in an early intervention program working with children (birth - 3 years of age) and their families. This position is responsible for the development & monitoring of Individual Family Support Plans related to the provision of Early Intervention Services for infants/toddlers with developmental delays & other special needs. Will provide time sensitive information & coordinate early intervention services. Applicants must be able to multi-task, prioritize work load, & are able to use personal computer. Services are provided in children's homes & in the community. Desired: Experience providing Case Management/Care Coordination in an early intervention or similar setting. Requirements: MSW, excellent organizational skills, valid driver's license, & vehicle that is insured & has had HI safety inspection .
